Registration for the 2012 Football & Cheer season is now open!!! Don't miss your chance to join TPPW, home of the 2011 National Champion Jr. Midget Falcon Cheer Squad.
For all registered players, we need:
1. Copy of legal birth certificate or passport (this must be a state- or county-issued, certified birth certificate, not a hospital certificate).
2. Wallet size school photo clearly showing the player's face.
3. The signed Participant Contract you printed out at the completion of your on-line Pre-Registration.
4. The registration fees are paid by credit/debit card at the end of the on-line registration. No checks or cash accepted.

Participant Contract
As mentioned above, TPPW will need 1 copy of the 2012 Player Participant Contract that you can print out at the end of your on-line pre-registration. This must be brought in to one of the in-person registration dates.
Physical Exam Form 2012
On or before June 30th, TPPW will need 1 copy of the Physical Form - so please have your physician sign the original document and make 1 additional copy, keep one for your records.
Remember, the physical must be completed on our Pop Warner form. If your doctor has performed a physical, as of January 1, 2012, you can have that information transferred to our form. You do not need a 2nd physical.
Need A Physical?
You can get a "walk in" sports physical at PPH Expresscare, located in Albertsons/Sav-on at 14340 Penasquitos Dr. SD 92129. Last year the cost was $30.

Appointments are not necessary and most visits take about 20 minutes. For your convenience, pagers are available so you may shop, visit the snack area, complete your banking and/or Albertson's customer service needs until the Nurse Practitioner is available to see you.

PPH expresscare is closed Monday-Friday from 1:30-2:30 PM and Saturdays and Sundays from 1:30-2:00 PM for required lunch breaks. The last patients are accepted 30 minutes before closing time each evening.
